These Terms of Use (the "Terms") govern your use of americanhealthcarehub.com (the "Hub") and its content, operated by Flex Stream, Inc. ("Flex Stream," "we," "us"), a California corporation. Use of the CareOnboard platform at staff.americanhealthcarehub.com is additionally governed by your agency's service agreement, including any Business Associate Agreement; if those documents conflict with these Terms, the service agreement controls for Platform use.
By accessing the Hub, subscribing to alerts, or purchasing a subscription, you accept these Terms on your own behalf and, where applicable, on behalf of the agency you represent.
News articles, alerts, and guides summarize regulatory developments from sources we monitor (CMS, CDPH, CDSS, ACHC, and others). They are informational only and are not legal, clinical, or billing advice. Regulations change; while every alert is cited and dated, you are responsible for verifying requirements against the primary source and your own counsel before acting. No affiliation with or endorsement by any agency or accreditor is implied.
Policy templates, forms, and other library documents are licensed, not sold. A paid subscription grants the subscribing agency a non-exclusive, non-transferable license to download, adapt, and use current versions of library documents for its own internal operations at the locations covered by its subscription. Library documents may not be resold, redistributed, published, or shared outside the subscribing agency. Templates require adoption and review by your agency before use; they are starting points, not a guarantee of survey or audit outcomes.
You are responsible for the accuracy of the information you provide and for keeping account credentials secure. Paid plans are billed per agency, monthly, and may be canceled at any time, effective at the end of the current billing period. Launch rates are honored per their stated lock period. We may suspend accounts for non-payment or material breach with notice.
You agree not to misuse the Hub or Platform: no unauthorized access or scraping of subscriber-only content, no interference with the service, no use that violates law, and no submitting information you lack the right to submit.
The Hub, Platform, library, and all associated content and marks are owned by Flex Stream or its licensors. Except for the library license above and personal, non-commercial reading of free content, no rights are granted.
The Hub and its content are provided "as is" without warranties of any kind, express or implied. To the maximum extent permitted by law, Flex Stream's aggregate liability arising out of the Hub or these Terms is limited to the amounts you paid us in the twelve months before the claim, and neither party is liable for indirect, incidental, or consequential damages. Nothing in these Terms limits liability that cannot be limited under applicable law.
These Terms are governed by California law; venue lies in the state and federal courts of Los Angeles County, California. We may update these Terms by posting a revised version with a new "Last updated" date; continued use after the effective date constitutes acceptance. Questions: hello@americanhealthcarehub.com.
